Inhaled Insulin Approved

The FDA has approved inhaled insulin. See the news.

We had a number of patients who volunteered to be in the studies that led to this approval. They felt they would return to the program they were using prior to going on the study and would not use inhaled insulin after its release.

Struggling with Byetta

This is the best “piece of work” I have seen in someone with type 1 diabetes starting the struggle with Byetta.

I've added a graph to my spread sheets so that it is easier to identify any trends. I notice that the curve of glucose readings is:
* High in the morning...dipping during the midday ...and rising until I inject byetta at the evening meal.
* I have noticed short periods of time during several of the days in January where the glucose level will "stabilize" for several hours at a time. It isn't exactly in the "normal range" but will be within 25 points for 3 hours at a time... its different behavior than what I've witnessed in the past.several examples are on the following dates:
jan 4
jan10

I experimented with the timing of the bolus in relationship to the byetta injection and found:
* I originally delayed my insulin bolus both in 1/2 hour and 1 hour delays after the byetta injection and found that my glucose shot up and did not retract easily. it stayed elevated even after a large delayed bolus. (jan11 noon-7pm)...almost like I "self regulated" at an elevated glucose level when I was anticipating that it would be falling.
* Consequently, I found that if I take the byetta and bolus and start eating within 10 minutes, my results are quite different. If the bolus from the pump is there during the first 10 minutes of byetta and eating, the results are that my glucose does not rise after eating but will fall and stabilize at an arbitrary point (7-midnight on Jan 4)
* types of food eaten matters...high carb intake with a byetta inj and bolus is better than low carb w/ byetta and bolus....complex carbs the best...fruits , whole grains...(jan12..all readings within 106 pts throughout the day) ...pizza always gives a delayed rise in glucose after the fats in the cheese and toppings is metabolized...readings spike 2-3 hrs later...today the readings have been all over the place and don't make any sense again...same curve but very elevated for unknown reasons....

I am going to try the dual wave approach this weekend where I spread my bolus out over a hour or two coinciding the start of the byetta injection.

I will continue to cut my background basal rate in small increments.
